School & Sports Physicals
High school athletes are required by Kansas law to have a physical examination before participating in sports and many states also request a sports physical for middle school athletes. The American Academy of Pediatrics supports the preparticipation sports exam.
We feel physicals are important too.
We make it easy! You can schedule an appointment with our primary care practice or a same-day appointment at Express Care, or simply stop by the Express Care office for a first-come-first-serve exam.
For School & Sports Physicals, a legal guardian must be present. Please bring all necessary paperwork & payment.

Kansas State High School Activities Association Physical Forms
To be eligible for participation in interscholastic athletics/spirit groups, a student must have on file with the superintendent or principal, a signed physical form. The annual history and the physical examination shall not be taken earlier than May 1 preceding the school year for which it is applicable.
